Do you sell 1099 templates in Microsoft Word or other format?

I am looking for a template for use with 1099 MISC income forms.  I purchased a package of Tops forms from Office Depot but cannot locate a template. I am familiar with Microsoft Office, Adobe PDF and OpenOffice, a template in any of these formats will be OK for me.
